PRIORITY VIOLATION-5-202.11 (A), P: Approved System and Cleanable Fixtures-- Observed Y-valve or side kick attached to mop sink faucet that has an AVB; one side was connected directly to the chemical dispenser and one side was connected to a hose that was hanging in the mop sink. Explained to PIC that the AVB vault is not designed to be continuously under pressure and that there shouldn't be any obstructions downstream from the AVB vault. This could result the in the backflow prevention device to fail. PIC called maintenance to disconnect the Y-valve. Recommended putting in a direct water line for the chemical dispenser.
