At start of inspection, observed customer with dog in store. Per employee by customer and dog, they had not approached customer to inquire on service animal status of the pets. Instructed manager on proper procedures for approaching customers and what questions are allowed to be asked of customers with pets. Corrective Action: Corrected at time of inspection.

Inspector's notes
Observed pipe from prep sink in the produce department below the floor drain at the time of inspection. An air gap of 1 inch above the floor drain is required for backflow prevention. Also observed leaking AVB on the mop sink by the produce room. All plumbing must be maintained in good repair at all times. PIC put in work orders for both pluming items at the time of inspection. Corrective Action: Correct prior to next routine inspection.

Inspector's notes
Observed a hanging sticky fly trap in the produce prep area at the time of the inspection. All fly catching equipment should be self contained to prevent contamination of food and food contact surfaces. Trap was thrown away at the time of the inspection. After discussion with the person in charge, it has been determined that the root cause of the violation was training staff which resulted in the violation. Corrective Action: Corrected at time of inspection.
