Observed peeling ceiling tiles over prep surfaces and a sink not mounted and caulked to the wall. Discussed adding cove base to all of the base floor junctures. Also observed a small roof leak near the hand-washing sink. Establishment to replace the ceiling tiles, mount and caulk the sink, place cove base, and repair the leak. Physical facility must be maintained in good repair at all times. Repair physical facility prior to next routine inspection. Corrective Action: Correct prior to next routine inspection.

Observed hand sink was no longer installed by three compartment sink. As per the person in charge there was leaking and sink was unable to be repaired. At time of inspection staff were using food prep sink to wash hands. Food prep sinks may only be used for preparing foods. Limited prep is completed on site, some produce washing and thawing of meats. Staff may use the sink basin for sanitizing as food prep until new hand sink is installed. Sink basin must be washed, rinsed and sanitized prior and after use as food prep. Corrective Action: Correct prior to re-inspection. Reinspection to check on replaced sink to be completed on or before Aug. 1, 2025

Observed ground beef in three door refrigerator with a temperature of 44-45*F. Cold hold of food to be 41*F or lower. Ground beef was placed in back corner of refrigerator and cooled to 43*F during inspections. As per owner, staff had been in and out of refrigerator for orders, and refrigerator had been restocked. Advised staff to monitor cold hold temperatures using a metal stem thermometer. Corrective Action: Corrected at time of inspection.
