Observed cold brew sitting in a dispenser on the center island of the market at room temperature, internally at 63*F. Time/temperature control for safety foods should be cold held at 41*F or lower to prevent the growth of pathogens. PIC stated the cold brew had been out for just over an hour and placed the cold brew under time as a control to be discarded at the 4 hour mark. Final construction inspection for a Class 2 10+ E&D permit was conducted with the construction team by appointment. The establishment was closed and not operating at time of inspection. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. Please note that if you select payment via online, the annual operating permit fee invoice may take up to 7 business days to post online. Also, checks and money orders are accepted via US Mail. Please address the following items prior to the completion of the project: 1) Provide an ASSE 1013 reduced pressure backflow preventer upstream of all glass rinsers. Certify and provide a test report. 2) Ensure all bar countertop/wall junctures are caulked or flashed off.3) Ensure no flex drains are installed at the bar. All drain lines must be rigid piped. The submitted Dog Friendly Patio permit will be approved on-site. The following items will need to be addressed prior to approval: 1) A separate entrance shall be provided from the exterior of the food establishment to the outdoor patio so that a dog will have direct access to the patio without entering the interior of the Food Establishment or any playground area of the Food Establishment. A dog on an outdoor patio shall not be allowed within seven feet of any entrance to the interior of the Food Establishment, except when necessary to enter or exit the patio. 2) A sign with at least 1/2 inch letters shall be posted at the front entrance of the Food Establishment so that it is easily visible to the public. The sign shall state: "Dog Friendly Patio - Dog Access Only through Outdoor Patio. For violations, contact MCESD (602) 506-6616." 3) No food may be prepared, including mixing drinks and serving ice, in the outdoor patio area, except that a beverage glass may be filled from a pitcher or other container that has been filled or otherwise prepared inside the Food Establishment. 4) Dogs shall not be allowed to have any contact with reusable food service dishes or utensils. A dog may only have contact with disposable single service containers that provide Food or water to the dog. 5) All patio surfaces shall be constructed of materials that are smooth, easily cleanable, and durable.
