TCS foods must be cooled in: uncovered/loosely covered, shallow containers less than 4 inches deep, in equipment designed to rapidly cool, by using an ice bath, by using ice as an ingredient, or another effective means. Observed food items, such as cooked carrots 5/19, raw pastrami 5/14, and raw, andouille pork 5/19, in reduced oxygen packaging past the 48 hour mark and not on the establishment's approved food list for their HACCP. Observed the establishment had not recently been filling out their temperature logs as required per their HACCP Plan. Following all steps on the approved HACCP plan, for the approved food list, is crucial to prevent the growth of pathogens. The PIC stated they have recently had a change in leadership for Le Ame, as well as some menu item changes. Due to this, updates to their HACCP plan may be required and the PIC will need to contact the HACCP/Variance dept. to make those changes. In the meantime, the PIC would like to move to the 48 hour date-mark for all of their Reduced Oxygen Packaged food and will be retraining their staff on the HACCP plan and it's requirements. Final construction inspection for a Class 2 10+ E&D permit was conducted with the construction team by appointment. The establishment was closed and not operating at time of inspection. This establishment/operation has demonstrated sufficient compliance to proceed with the issuance of the permit. To complete the permitting process, issuance of this permit, and begin operation, the applicant must first submit payment of the annual operating permit fee to the Department’s Administrative Services. Fees paid up to this inspection approval do not include the annual operating permit fee. PAYMENT with a credit/debit card can be made via phone at 602-506-6824, Option 2. Per MCEHC Chapter I, Regs 4 & 5, no person shall conduct an operation or operate an establishment for which a permit is required without holding the necessary and valid permit to do so. Payment with a credit/debit card can be made on-line via select “Pay a Fee” at esd.maricopa.gov. The establishment was closed and not operating at time of inspection. The permit is not approved. Please address the following items prior to the final construction inspection: 1) Plumb the countertop drain board near the undercounter warewash machines to the nearest floor sink and provide a 1" minimum air gap. Ensure rigid piping is utilized. 2) Extend the handwash sink drain at the banquet bar to ensure waste does not overflow on to the floor. 3) Install an ASSE 1013 reduced pressure backflow preventer upstream of the RO system installed for the espresso machine. Certify and provide a test report. 4) Provide the addtional lighting in the kitchen, as discussed. 5) Continue caulking escutcheon plates and plumbing penetrations, as needed. 6) Grout the top gaps of the coved tile in the walk-in coolers. 7) Provide missing dome grates, as needed. Certify and provide a test report. 2) Provide an ASSE 1013 reduced pressure backflow preventer for the pasta cooker or provide a faucet with a 1" minimum air gap above the flood rim. Certify and provide a test report. 3) The flex drains for the Combi ovens is not approved. Provide a rigid piping and provide 1" minimum air gaps above the floor sinks. 4) Provide 1" minimum air gaps above the sink flood rims for the sprayer facuets. 5) Replumb or trim the indirect drain lines in the kitchen to reduce the splashing at the floor sinks. 6) Increase the lighting in the kitchen, as discussed. 7) Regrout the tile flooring throughout the facility to ensure the grout lines are flush and smooth with the tile. 8) All air curtains throughout the facility must have all manual switches disabled and operated solely by a micro-switch. 9) Provide a splash guard to the right side of the handwash sink located in the service corridor.
