Observed several items in cold holding unit with internal temperature ranging 43-53*F (Lettuce, shredded cheddar, sour cream, yogurt, cottage cheese, boiled eggs). Yogurt and cottage cheese ranging 43-44*F were placed in unit within 1 hour of inspection and put in neighboring unit to reach 41*F and below. Remaining food items were placed in unit over 3 hours prior to inspection and were voluntarily discarded. Observed mechanical warewashing machine run several cycles with dirty dishes, and produce a chlorine strength of less than 50ppm. Unit should produce a strength of at 50-100ppm every time for proper sanitation. Person in charge stated a service request has been placed, and will be completed today. Staff removed dishes, and began to prepare 3-compartment sink for manual washing. All dishes to be washed and sanitized manually at 3-compartment sink until dishwasher is able to produce a chlorine concentration of 50-100ppm. After discussion with the person in charge, it has been determined that the root cause of the violation was misread of test strips during testing, which resulted in the violation. Foods in refrigerator (cheeses, sauces, cooked vegetables, stuffed pasta, raw salmon, gravies, raw meatballs, etc) in reach in refrigerators with an internal temperature of 48-50*F. Items date marked for 8/31 were prepared within 4 hours prior to inspection (raw meatballs, raw salmon, gravies, etc.). Foods date marked for 8/31 were placed in walk-in refrigerator in bistro and reached an internal temperature of 41*F within 1hr and 15min. Person in charge mentioned other foods were placed in the unit over 4hrs prior to inspection. All cold temperature controlled foods should be held at 41*F or below. Person in charge voluntarily discarded (see embargo list). After discussion with the person in charge, it has been determined that the root cause of the violation was limited verification of temperatures throughout the day, and improper diagnosis from kitchen staff, which resulted in the violation. Recommended increasing frequency of documentation of temperatures throughout the day and checking temperature logs before utilizing refrigeration units.
