Cut lettuce-46-47F cooling in a reach in cooler in the rear prep area. PIC stated that lettuce was prepped over four hours prior and discarded the lettuce-see embargo form. PHF/TCS foods prepared from ingredients at ambient temperature shall be cooled within 4 hours to 41°F or less. Corrective Action: Corrected at time of inspection.

PRIORITY VIOLATION-5-202.11 (A), P: Approved System and Cleanable Fixtures-- Observed chemical dispenser directly connect to mop sink faucet. Explained to PIC that the AVB vault is not designed to be continuously under pressure and that there shouldn't be any obstructions downstream from the AVB vault. This could result the in the backflow prevention device to fail. Had PIC disconnect chemical hose from the mop sink faucet at time of inspection. Please install a dedicated water line for the chemical dispenser prior to next routine inspection. Correct By: Corrected At Time Of Inspection
